All Services

Available to individuals with disabilities of all ages and are offered in: Albany, Schenectady, Rensselaer, Saratoga, Columbia, Greene, Warren, Washington, Fulton, and Montgomery Counties.

Individualized Community Services (formerly known as Community Supports Services) are provided by our experienced staff to individuals either in their home or in the community.

We work with the individual and the family to develop a unique plan designed to assist in building life skills such as:

  • Household management
  • Activities of daily life
  • Development of play, social, and leisure skills
  • Community integration and involvement
  • Personal care
  • Safety skills

In many instances, our staff work with individuals to help them design their own services such as; selecting service providers, choosing supports, determining how to use funding, and coordinating schedules. This gives individuals more control and flexibility to choose the mix of supports and services that work best for them, so they can live the life they desire. It also empowers individuals by providing them the opportunity to manage their services and supports based on their personal strengths and needs.

This all results in individuals being able to:

  • Enjoy meaningful relationships with family and friends
  • Experience health and growth
  • Live in the home and community of their choice and
  • Work, volunteer or do activities they enjoy
