About our HARP/HCBS Services:

Our team of Community Service Specialists focus on behavioral health (mental health and substance use). Our team will listen and help you develop a personal plan to pursue what’s important to you.

With years of experience in providing community based person-centered services, our Community Service Specialists have developed the knowledge and abilities necessary to assist you in obtaining or maintaining your independence and self-sufficiency.


We provide HARP/HCBS services throughout the greater Capital District, in the counties of Albany, Rensselaer, Saratoga and Schenectady.

Our Services

  • Person-Centered Care – Designed to empower you, provide choice, and reflect your personal goals.

  • Recovery-Oriented Care – Founded on the belief that all individuals are able to overcome challenges related to mental illnesses and/or substance use disorders and reach personal life goals. We provide services in your home and community in an effort to promote hope and encourage you to establish your own path towards recovery.

  • Integrated Care – Intended to address both your physical health and behavioral health needs. We work with your team of providers to best support your individual goals and foster personal responsibility for health awareness.

Our trauma-informed Community Service Specialists work with you to support and engage family members in your recovery process.


Our experienced Community Service Specialists provide you with person-centered, strength-based trainings to establish and promote skills of daily living.


Our Community Service Specialists assist you in developing behaviors and skills that foster community integration and maximize your independent self-management.
