Community Habilitation Services

 

Community Habilitation Services are home-based support services designed to assist individuals and their families.

Examples of these individualized services may include, but are not limited to:

• Household management
• Activities of daily life
• Development of play, social and leisure skills
• Community integration and involvement
• Personal care
• Safety skills

Community Habilitation Services are available to all age groups. Services may be provided in home, community or program settings as needed.
