TBI Service Coordinator

Catholic Charities Disabilities Services (CCDS), a leader in services for individuals with developmental disabilities, has a full time opening for a TBI Service Coordinator position, within our Individualized Community Supports department.

Job Summary: The position of TBI Service Coordinator provides service coordination to individuals and families residing in the community at large. In general, the services entail helping individuals and families access appropriate programs, benefits and other opportunities that improve their quality of life.

Location: Albany, NY

Responsibilities include; but are not limited to:

  • Facilitates the Initial Service Plan (ISP) and waiver program eligibility.
  • Coordinates multiple services among multiple providers.
  • Secures initial and annual level of care assessments.
  • Assures that Team Meetings are scheduled and held as designated in the service plan.
  • Facilitates the acquisition, oversight, and delivery of service.
  • Ensures annual service plans (Revised Service Plans/RSPs) are completed in a timely manner.
  • Facilitates the waiver applicant/participant’s Plan of Protective Oversight is completed and supports the service plan.
  • Conducts monthly face to face visits and in-home visits with the participant no less than once a quarter.
  • Maintains records for at least six (6) years after termination of waiver services.
  • Responds to participant crises and emergencies.
  • Addresses problems in service provision.
  • Actively participates in agency activities that are intended to enhance teamwork and morale.
  • Demonstrates knowledge and ability to perform basic math, including percent and/or ratios; write documents following prescribed formats, as well as present information to others.
  • Maintains current knowledge base of DDRO/OPWDD regulations and DOH policies, procedures and regulations related to service coordination.
  • Adheres to all guidelines and regulations set forth by the agency’s business plan(s) and/or other oversight entities such as, but not limited to; OPWDD, CDC, DOH.


  • A Bachelor’s Degree in Human Services or a related field is required; Master’s Degree is preferred.
  • Three years’ experience with Bachelor’s degree or one year experience with a Master’s degree of case management providing: case management/service coordination, information and referral services and service brokering of community based services for individuals with disabilities and/or Traumatic Brain Injury.
  • May be required to work extended hours or a flexible schedule with short notice.
  • A valid NYS driver’s is required.
