TBI Service Coordinator

We are currently hiring for an TBI Service Coordinator, to work within our Catholic Charities Disabilities Services agency. As an TBI Service Coordinator, you would be responsible for:

  • Providing service coordination to individuals and families residing in the community at large.
  • Maintaining current knowledge base of DOH policies, procedures and regulations related to service coordination.
  • In general, the services entail helping individuals and families access appropriate programs, benefits and other opportunities that improve their quality of life.

TBI Service Coordinator Minimum Salary: $20.51/hour

TBI Service Coordinator Maximum Salary: $22.51/hour

TBI Service Coordinator M-F 8:30am-5:00pm; Combination of in-office and remote work.

Qualifications

  • Valid NYS Driver’s License
  • A Bachelor’s Degree in Human Services or a related field is required; Master’s Degree is preferred. See below for details:
    • Bachelor’s degree and three years’ experience providing case management/service coordination for individuals with disabilities and/or Traumatic Brain Injury
    • Master’s degree and one year experience of case management providing case management/service coordination for individuals with disabilities and/or Traumatic Brain Injury.
