Community Supports Professional

Catholic Charities Disabilities Services (CCDS), a leader in services for individuals with developmental disabilities, is now hiring for a part-time Community Supports Professional (CSP).

Location: Capital Region

Job Summary: Community Supports Professionals provide direct services in the home and/or a community setting, to individuals with a developmental disability. Through community events and exploration, Community Supports Professionals work one-to-one with people with disabilities on goals that increase that person's independence, such as, healthy living, socialization skills, money management, and more. The objective of the Community Supports Professional is to contribute to a positive living environment and to enhance the quality of life for the individual being supported. This position is part-time and offers flexible schedules.

Responsibilities include; but are not limited too:

  • Meet the needs of the individuals as described in the Community Habilitation Plan and attend all mandatory meetings and trainings as required. This includes personal care training, activities of daily living, recreational activities, travel, etc.
  • Communicate with individuals, family and supervisory staff in meeting the needs of the individual by providing a high quality level of service to him/her in accordance with the Community Habilitation Plan.
  • Successfully participate in education and mandatory training requirements in a timely manner as considered appropriate by the Community Supports Supervisor and/or QA/Training manager.
  • Establish working hours as agreed upon by the individual primary care provider, family and Community Supports Supervisor.
  • Attend and participate in all meetings as deemed appropriate by the individual, family and supervisor. This may include meetings with the individual and his/her primary care provider, supervisory meetings and mandatory staff meetings from time to time.

Qualifications:

  • High School Diploma/GED required. 
  • Valid NYS driver’s license and reliable vehicle (mileage reimbursed).   
  • Smart Phone and/or Tablet is required for electronic record keeping.
  • Ability to work independently, demonstrate sound judgement and complete necessary documentation.

Skillset:

  • Must have capability to utilize different web applications for completing record keeping electronically.
