Service Coordination Catholic Charities Disabilities Services' dedicated team of Service Coordinators will assist you in determining what services you may be eligible for and how to access them. Service Coordination services assist persons with developmental disabilities and their families in gaining and maintaining access to services and supports appropriate to their needs. Service Coordination services are provided by qualified service coordinators and use a person-centered planning process in developing, implementing, and maintaining an Individualized Service Plan (ISP). Service Coordination provides a vital link to information about service options. Catholic Charities Disabilities Services has a dedicated team of Service Coordinators who can assist you in determining what services you may be eligible for and how to access those services. We will assist you with navigating through available supports and services; completing applications; advocating for yourself; obtaining adaptive devices for your home; guardianship; family support funding; health, medical, and clinical services; and obtaining or maintaining residential supports and services.
