HARP HCBS Provider

Location: Capital District, location can vary depending on the person receiving services, and the successful candidate will be open to cases anywhere within the tri-city area of Albany, Schenectady, and Troy

Position Description: 

Under Medicaid Managed Care, Health and Recovery Plans (HARPs) offer Adult Home and Community-Based Services (HCBS) to eligible adults with mental illness and/or substance abuse disorders.  HARP HCBS services provide enrolled individuals the specialized supports they need to live independently in their community, and aid recovery.

 

The role of the HARP HCBS Provider is to support enrolled adults in attaining goals and objectives uniquely identified by each person receiving services, according to their Plan of Care.  Some examples of the types of skills areas the HARP HCBS Provider might assist with; include, but are not limited to the following: relapse prevention planning, social skills development, wellness and self-management, Activities of Daily Living (ADLS), building community resources, money and time management, and/or providing education/support to family.

Qualifications:

  • A high school diploma or a GED/HSE is required, Associate’s level course work is preferred.
  • At least one year of experience working with individuals with substance abuse or mental health diagnoses is required.
  • A valid New York State driver’s license and reliable transportation is required.
  • Basic computer and internet navigation skills are required.
