Service Coordination
Catholic Charities Disabilities Services has a dedicated team of Service Coordinators who can assist you in determining what services you may be eligible for and how to access those services. Your service coordinator can help you with:
- Obtaining the services you need
- Program eligibility determination
- Advocacy
- Emotional support
- Referral and Coordination of services
- Arranging temporary relief such as respite in and outside the home
- Assistance dealing with various agencies including social services, schools, etc.
- Locating out of home placement if desired
- Obtaining equipment and supports
